General Medications

The most common way to get a refill on your prescription is by having the local pharmacy fax our office a request for refills. The doctor will respond within 24 hours to the request.

If you require a new prescription for a new medication and have discussed this change in medicine with Dr. Guarino, please call us at the office at (408) 356-4777 and provide us with your local pharmacy's phone number, so that we can call in the prescription.

Note: Both of these methods apply to medications that are not controlled substances.

Controlled Substances

IMPORTANT NOTE: If you need a refill on a controlled substance, a stimulant that can be potentially abused and may lead to possible psychological or physical dependence, please call one week prior to your triplicate medication(s) running out for a refill. Dr. Guarino and his office staff need time to pull your chart and review your notes for the prescription to be written out. Please allow 24 hours for a return call to inform you of the prescription being ready for pick up, or if it is being mailed out to you.

These prescriptions CANNOT be phoned into or faxed to the pharmacy. Thank you for your cooperation.

The following is a list of the affected medications:

  • Adderall
  • Concerta
  • Daytrana
  • Dexedrine
  • Diastat Acudil
  • Focalin
  • Metadate
  • Methylin
  • Ritalin
  • Vyvanse

Note: Under the Controlled Substances Act enforced by the DEA, patients on these medications require subsequent check-ups with the prescribing doctor to monitor your child's health. Failure to make these appointments is a violation of the doctor's standard of care and no refills will be authorized by the doctor, until your child comes in for the visit.

Refills Through Mail-Order Pharmacies

If you are using the mail-order pharmacy for the first time, please provide us with the following information:

  • Your pharmacy member ID (if necessary)
  • An order form with the pharmacy's fax number
  • Any specific instructions on how to write the prescription (i.e. 90-day supply only)
  • Any necessary information that must be included with the order

If you require a refill on a medication using your mail-order pharmacy, please have the pharmacy fax a refill request form to us. The doctor will attend to the request within 24 hours.
